In order to allow our students to be administered medicine during the school day permission must requested and approved by the parent and approved by a physician.  Nurses should print the attached letter on your school letter head and obtain the appropriate signatures before giving the form to the parent to finalize the approval.  The instructions on the letter are also in Spanish for our Spanish speaking parents.
